W-9 & W-8BEN Forms

Dear LivePerson Expert,

Under U.S. federal tax law, LivePerson is required to request certain taxpayer information from certain persons receiving certain funds from LivePerson (whether such persons are U.S. taxpayers or not). Information collected will be used solely to fulfill LivePerson's requirements under U.S. federal tax law and will not be used for any other purpose.

In order to fulfill this requirement we must receive immediately your completed W-9 or W-8BEN form, as applicable, declaring your tax status.

Please note that a failure to submit your tax form may force us to turn over (withhold) 28% (or 30%, as applicable) of your gross earnings on a monthly basis directly to the IRS. This process will continue until such time that we receive your completed W-9 or W-8 form, as applicable.

US Taxpayers (Form W-9)
If you are a US taxpayer (for example a US corporation, a US partnership, a US citizen or a US resident alien) you should submit W-9 Form.

Non-US Taxpayers (Form W-8BEN)
If you are not a US taxpayer, you should submit W-8BEN Form.

Please note:
It is important that you complete the applicable form and send it to us immediately. You may sign and scan your form and email it to us at forms@prestoexperts.com

Most importantly, please make sure to write your LivePerson Member ID (can be found in your "Account Details" section) in the top right hand corner of the form to assist our processing.

Thank you for your prompt attention to this matter.
